then when we went in they asked something about if I worked and was the insurance through my work and something about last names that sounded fishy as if I was a single parent with 2 kids. I can't recal the wording but I thought it quite an odd question -- but considering the welfare/insurance comment over the phone that maybe considering the neighborhood they are in that they maybe deal with ALOT of single women on welfare.
So they want me to update a file for Sarah - here is part of the form they gave me to fill out!!! I laughed because it fit right in with the previous questions of welfare and now Im feeling that everyone who comes in to get glasses must be poor and on some sort of drugs.
I guess I always thought medications and prescriptions were the same thing - so the question to me looks like they are asking which prescriptions and drugs are you taking. I woulda answered: yes and Heroin - just to see the look on their faces but, because of the sensitivity of the area of town we were in I didn't want to have cops called. :eyeroll: :goofy: :eek: :chagrin:

I filled out something close to that at my eyedoc too - it's pretty much a standard consent to treat form.
Also, certain diseases (like diabetes, high blood pressure, stroke) can affect your vision. which would explain the medical history part.
I don't think your eyes ever quite recover from that.

funny (well at the time it wasn't) story.
When I got arrested a couple years ago, when they were booking me, they asked me what drugs I was on. I told them that earlier that day I had taken 400 mg of Ibprofin for my lower back, and 1000 mg of vitamin C.
After the sherriff who was doing my intake stopped laughing, and telling the 4 other sherriffs around her - how stupid I was, she explained that she meant NARCOTICS.
